Did you know? Oakwood Creative Care’s Dementia Hub is Arizona’s first-ever all-inclusive approach to improving care for people living with dementia and their care partners. Today, The Dementia Hub helps families pave a path that begins with a loved one’s diagnosis and continues along their entire journey with Alzheimer’s or another form of dementia…preserving a meaningful life with their loved ones and celebrating each moment of joy along the way. Helping us in this mission is Oakwood’s new Medical Director, Dr. Mari Mercado!
Get to Know Dr. Mari, the New Dementia Hub Medical Director:
Dr. Mari Mercado is a bilingual, board-certified Family Medicine physician who has been practicing in the Phoenix, AZ, region since 2008. She graduated from the New York College of Osteopathic Medicine and completed her residency at the University of Oklahoma—Tulsa Family Residency Program.
Dr. Mercado has a subspecialty in OB-GYN and was a Public Health Nutritionist before becoming a physician. Her passions are healthy aging, nutrition, and wellness. Her commitment to her field is further demonstrated by her role as the board president of the Arizona chapter of the American Academy of Family Physicians.
We are so excited to welcome Dr. Mari into the Oakwood family! Her compassion and years of expertise are bound to make a tremendous difference within our community, especially as we implement the new GUIDE Model Program.
The GUIDE program was spearheaded by the Centers for Medicare & Medicaid Services (CMS) to improve the quality of life for people living with dementia, reduce strain on caregivers, and help people remain in their communities. GUIDE offers care coordination, caregiver education and support, and respite services.
As Medical Director for the Dementia Hub, Dr. Mari will be giving a special introductory presentation for GUIDE on Thursday, September 19th, from 3:00 – 4:00 PM. This is a free event that will be open to everyone in the community and especially our fellow medical providers.
